Add CODE_OF_CONDUCT.md at the repo root using the Contributor Covenant (v2.1) — it's the standard for most TypeScript/JS open source projects, so mostly an adapt-and-fill-in-contact-details task. Link to it from the README and CONTRIBUTING.md.
