fix(docs): enforce generated documentation

Summary

• make the root README canonical and keep the publishable package README synchronized
• add an affected Moon CI check that fails on stale generated README examples
• make README changes select split-doc generation and the final VitePress build
• route generated docs edit links and agent context to the canonical README

Verification

• stale generated-example probe fails, then cssforge:readme-update repairs it
• stale package README probe fails, then cssforge:readme-update repairs it
• README affected-CI selection runs cssforge:readme-check, docs:sync-docs, and docs:build
• unrelated affected-CI selection skips docs tasks
• Cloudflare build entrypoint passes with pinned Proto/Moon toolchain
• Wrangler static-assets deploy dry run reads 65 generated files
• independent standards and spec reviews are clean
