[Swift] Handle Cloudflare managed challenges

Summary

• detect Cloudflare managed challenge responses directly in CheckoutWebView
• allow visible checkouts and subframes to render managed challenges
• stop and evict hidden cached checkouts that receive main-frame challenges without emitting checkout lifecycle failures
• report discarded unconsumed preloads as HTTP failures while returning dismissed retained views to idle
