Drop codeclimate-test-reporter (unused since 2018), which pinned simplecov to 0.13; upgrade to simplecov 1.0. Report unit-test line coverage on the postgres:17 CI leg via job summary and an HTML report artifact. Switch the README maintainability badge to qlty and remove the stale coverage badge.
